Application for Main Street Community Living

Please complete all information listed on the application form. Once submitted, we will have a representative reach out to you to provide additional information. The following will be required for admission:

- Meeting with a Main Street Representative to discuss your substance abuse challenges, treatment and long-term goals

- Ability to pass a 72-hour alcohol and 13-panel drug test 

- Policy agreement and acknowledgement of program fees to be paid before admission.
